Application Forms for 2012

You can complete the following forms on-line by selecting the link.
On line
registration will help with the administration for Match Secretaries.
Forms can be completed online, but printed copies must be sent with
payment to

Please remember to include sufficient postage if using a large envelope!
All club coaches should have appropriate qualifications and evidence should
accompany all applications - photocopies of certificates. Clubs who fail to do
so will not be accepted into membership of the Association.
New coaches have 6 months to achieve qualifications. Coaches named on previous
applications must have these qualifications as their 6 months probationary
period has expired.
Clubs joining ESSDA must also be Associate Members of SYFA.
SYFA Membership is an online process from
www.syfaregistrations.co.uk
SYFA Membership runs from August to June, so membership does not require to be
renewed until June 2012. It can be renewed earlier.
eMail addresses will be used by ESSDA to contact coaches and club officials to
keep them informed of ESSDA business and may be given to SFA and/or SYFA for
similar purposes.
eMail addresses will not be given to 3rd parties, but with consent will be given
to other ESSDA coaches.
- Application Form for Season 2012 (Soccer 7s)
The completed form should be printed and sent to ESSDA together with copies of
Coaching and First Aid Certificates and payment of £20 per team (£10 for
2000 12s).
The entered details will be emailed to your Match Secretary, but the
accompanying paperwork must also be completed.

Coaching and First Aid Certificates and payment of £15 per team.

SYFA Registration is not required until summer 2012.
